Get the Surplus Sales Bid Sheet form. |
- Click Surplus Sales Bid Sheet (PDF), and print out a copy of the form.

Complete the Bidder Information section of the form. |
- On the left side of the form, list your:
- Full name
- Home address, city, and state
- Home telephone number
- E-mail address
- Resale number (if any)
- If you are a UCSD employee, fill in the following information in the box marked UCSD Employees Only:
- List your employee identification number.
- List your department.

Complete the Items Desired section of the form. |
- Provide the following information, which you will find when you go to the UCSD Surplus Sales Web site and/or when you visit the Surplus Sales facility:
- The lot number
- A brief description
- List your bid amount.
Bid recommendation:
- Bid the price you are willing to pay for the item.
- Examine, test, or evaluate all items before placing a bid.

Read the information in the Conditions of Sales section of the form. |
- While reading the Conditions of Sales, please pay close attention to the following:
- All items are sold on an "as is, where is" basis.
- There is no warranty on any items sold.
- All sales are final.
- All items must be removed within seven days of bid acceptance.

Sign and date the bottom section of the form, and review for legibility. |
- Sign your name on the signature line.
- Enter the date you complete the form.
- Review the form for legibility. If the form is not legible, your bid may be rejected.
