Controlled Substances: Applying for a CSUA  
 
Summary: Principal investigators (PIs), before acquiring controlled substances (CS) for research, teaching, or veterinary purposes, find out how to apply for a Controlled Substance Use Authorization (CSUA).

CSUAs designate authorized personnel, describe CS use, and record their inventory in compliance with strict federal regulations. To find out if you need a CSUA, see Determining if a Chemical is a Controlled Substance.

What to do How to do it
Step 1: Apply for a CSUA. First-time applicants, complete these 3 forms required for application and submit them to the EH&S Controlled Substances Office:
  • Form 1: Print and complete a Controlled Substance Use Authorization application form (6 pages). Ensure you complete these sections:
    • Section IIA: Review sample section IIA (PDF) for listing CS.
    • Section V: Your department chair must complete the "Department Chair Approval" information in section V, not the applying PI.
    • In vitro use: Include an In Vitro Protocol when using CS for in vitro purposes.
    • Animal use: Make sure you list CS used in animals in section 17 of your Animal Use Protocol.

  • Form 2: Print and complete a Controlled Substance Inventory form.
    • Complete the information at the top of the inventory form.
    • If you do not possess any CS, write "zero" in the "Number of completed line items in table" blank.
    • If you have CS in the lab, enter the name(s) and amount in possession.

  • Form 3: Print and complete a Personnel Screening Data Sheet for each individual whose job duties require access to the CS.

Additional authorizations or registrations: Depending on your purpose and CS schedule number, state and federal regulations may require you to simultaneously obtain other authorizations or registrations when you apply for a CSUA.

Step 2: Schedule a storage review meeting. Contact the Controlled Substances Program manager, (858) 534-1362.
  • When you complete all application forms and the storage review meeting, a CSUA approval letter, copy of the approved CSUA, and authorization number will be sent to the PI and primary lab contact.
  • Read about CS storage requirements.
Step 3: Review the CS Program. Review the Controlled Substances Program overview to ensure you understand CS Program responsibilities and procedures.
  • See Controlled Substances: Training. Principal investigators, their lab contacts, and authorized personnel working with CS are strongly encouraged to attend.
