Decide which customs form you need. |
- Form 2976, Customs - CN 22 Sender's Declaration is required on all international mail weighing 1 to 4 pounds.
- Form 2976-A, Customs Declaration and Dispatch Note is required on all international mail weighing more than 4 pounds.
If you use this form, you also need a 2976-E Customs Declaration Envelope.

Get the form and supplies you need. |
- Go to UCSD Mail Services Customer Supplies (PDF) and complete the order form to get your free USPS supplies. If you need assistance to complete the form, see How to Order Mailing Services Supplies.
Note: Most items will be delivered to you within 24 hours.

Complete USPS customs form 2976 (if applicable). |
- If you are using USPS customs form 2976, fill in the following informaton:
- A detailed description of the contents including:
- Value of each item
- Total value
- The type of contents
- Signature of sender
- Sender's name and full address
- Addressee's name and full address
- Sender's signature
- Current date
Note: Abbreviations are not accepted.
Important: The return address on your item and on the customs form must be identical.

Complete customs form 2976-A (if applicable). |
- If you are using USPS customs form 2976-A, fill in the following informaton:
- Sender's name and full address
- Addressee's name and full address
- A detailed list of the contents including the:
- Quanity
- Value for each item
- Net weight of each item, if known
- The type of contents
- Disposition of mail in case it is undeliverable
- Sender's signature and current date
Note: Abbreviations are not accepted.
Important: The return address on your mail and on the customs form must be identical.

Place the completed form on your mail. |
- The form should go on the bottom left corner of your mail item. If you are not sure how to prepare your mail, read how to send USPS international mail.
Reminder: A customs form doesn't replace your address label.

Mail your international mail |
- Put your mail with your outgoing mail.
