Hiring form More information
Resume Screening Worksheet (Word file) Use this form when reviewing applicants and selecting an interview pool. This document is optional to Central HR and may assist you in the event of an audit.
Note: Check with the Human Resources contact in your department to see if they require this form.
Staff Employment Selection Report (Word file) (Excel) This form is required for all new hires. Use it to document the interview process and outcome.
Staff Appointment Report (Word file) This form is required for Business Affairs units only. Include it with the Selection Report when you request HR approval for a recruitment.
Nurse Verification Forms These forms are required when you hire for any nurse or nurse practitioner position:

Use this form for career-position nurses who work in a specialty, qualify for a certification pay differential, and maintain current certification:

Reference Check Template (Word file) Use this template when you are conducting reference checks on your final candidate.
Resume Supplement (PDF) This form is required if an applicant does not complete the online resume/ application, or if you are completing a Short Term Exception hire.
Criminal Disclosure Form (PDF) This form is required if an applicant does not complete the online resume/ application.
