How to Enroll in Payroll Direct Deposit

Complete the Payroll Wage Disposition Request form. |
- Download a copy of the Wage Disposition form.
- Complete the form:
- Select Option I, Direct Deposit.
- Indicate where to deposit the money:
- Checking account: Attach a voided check.
- Savings account: Attach a copy of your bank membership card.
- Members of the University and State Employees Credit Union: Submit a copy of your bank membership card (deposit slips not acceptable).
- Provide bank information:
- Name of financial institution
- Account number
- Nine-digit routing number, listed at the bottom of your voided check on the lower left-hand side. You can also get this number from your bank.
- Sign and date the form.

Submit the completed form to Payroll.

Note: Direct deposit will take effect within eight weeks from the date Payroll receives this request. You will receive a paper check in the interim.
