Summary: UCSD Risk Management has several insurance programs covering shipments to or from UCSD. Before you ship a package, review this page to select the right type of insurance based on the destination and value of your shipment. |

UCSD offers two types of insurance programs:
- Automatic insurance which requires no enrollment forms.
- Underwritten insurance which requires an application, premium payment, and approval before coverage begins.
Note: Since each department is responsible for uninsured losses, be sure to check the limitations and exclusions of the insurance program you choose.
Automatic Insurance
- Domestic (continental United States and Canada - not including Alaska or Hawaii) shipments valued under $100,000 are automatically insured if you can verify the shipment value with a purchase order, shipping memo, bill of lading, or air bill invoice.
Underwritten Insurance
- Domestic (continental United States and Canada - not including Alaska or Hawaii) shipments valued over $100,000 require approval one week before the shipment date and cost $.05 per $100 of shipment value. To submit your request and get approval:
- Complete the Domestic Shipments Over $100,000 form (PDF) (Word file).
- Fax the form to (858) 534-5202.
- International/ foreign
All shipments going to Afghanistan, Angola, Ethiopia, Iran, Iraq, Nicaragua, Nigeria, Somalia, Uganda, and countries of the former Soviet Union and Yugoslavia require prior approval and a two-week notice.
- Shipments valued under $2 million require notice to Risk Management at least by the day of shipment and cost $.24 per $100 of shipment value. To submit your request:
- Complete the Foreign Shipment Insurance form (PDF) (Word file).
- Fax the form to (858) 534-5202.
- Shipments valued over $2 million require approval three weeks before the shipment date. Contact Scheryl Wade, (858) 534-4237, for an estimate of the cost. To submit your request and get approval:
- Complete the Foreign Shipment Insurance form (PDF) (Word file).
- Fax the form to (858) 534-5202.
Note: Requests submitted with less than a three-week notice cannot be guaranteed insurance coverage.
